Thank you for your inquiry to the WV LPN Board of Examiners. We offer ALL CURRENT ACTIVE WV LPN's: Name, Address, Email Address, State, License Status, Permanent, Temporary and License type. Our fee is $100.00 for this purchase. The WV State Treasurer's Office has a separate fee of $2.25, and your receipt will say WV State Treasury on it.  The​ total will be $102.25.

We accept VISA, MASTERCARD, AND AMERICAN EXPRESS. 
 

As of July 1st​ 2024 go to our new website: https://lpnboard.wv.gov then go to discipline tab on home page,  Hover over discipline, then click on page that says discipline payments

​​​1.  Where it asks for nursing license number type in 00000 (five zeros)
 

2.   ​Amount to Charge ($100.00) one hundred dollars; a fee will be added at the end by WV State Treasure's Office
 

3.   Reason: Active Mailing List​
 

once you complete this form: a receipt will be sent to the email you put in: Upload receipt to the board email: lpn.board@wv.gov or to victoria.l.carter@wv.gov​​ or fax it to 304-558-4367 for processing.
