Applicable Rules and Laws for Verification of Employment Hours
3.1.b.1. Each applicant for renewal of a license, if the license is currently active, or reinstatement of a license, if the license is lapsed or on the non-practicing list, shall verify that he or she engaged in four hundred (400) clock hours of practical nursing practice within the prescribed two (2) year year reporting period.
Verification of employment hours: Employer can send directly to us at the board via fax: 304-558-4367 or email: lpn.board@wv.gov The employer can submit directly to the board on their letterhead: It must include dates and hours, but does NOT have to be notarized if they send it. If they give it to the nurse to upload the forms below must be notarized.
