WV LPN Board Onsite Accreditation occurs every three years at every accredited program.  

Once you complete the form put a word doc and pdf version on the thumb drive, 

Please put documentation in folders that say: Goal 1, Goal 2, Goal 3, Goal 4, Goal 5, Goal 6, Goal 7: Then insert in each folder the documentation the Accreditation Form asks you for under each Goal.

​Annual Report 

Each Practical Nursing Program must complete Annual Report to the Board 30 days after completion of the program for each fiscal year.
