Per legislative laws LPNs are required to have:

400 hours of employment every 2 years on the even year.
24hrs. of continuing education and within the required must include: 2hrs veterans or mental health for veterans and their families and 3hrs substance abuse or drug diversion within those 24hrs (completed every two years on the even years)
For new nurses or new to WV must complete 2hrs of end-of-life care with pain management (a one-time CE)

To Start the Application Process for Licensure

Know where to go before logging onto your nurse portal:

Once you start an application in your nurse portal you will need to check your message center regularly for updates.

VETERANS:

If you currently hold a WV LPN license and you are on ACTIVE duty or have a Spouse On ACTIVE duty: You are EXEMPT from continuing education audit requirements, just upload a copy of your ORDERS from military to the nurse portal message center.

To challenge the NCLEX exam you must have an OFFICIAL DD214. You cannot challenge the NCLEX exam if you are active duty. Your training must be within three (3) years of your discharge date. Please click document below full details.

Employer to complete verification of employment (VOE) once nurse completes the 200 hours of employment with direct supervision by (RN, MD, Dentist).

NAME CHANGE APPLICATION:

You will need to log into your nurse portal and click on Apply for License: https://wvpn.boardsofnursing.org/

Click on OTHER Applications: Name change Fee is $10.00

You will need to upload court documents to support name change: Marriage Certificate, Divorcee Decree, or Name Change court document (It must state that your name is changed from and to.) All these documents must have an official seal visible on it.

INITIAL BY EXAM APPLICATION-FIRST TIME TAKING NCLEX

Initial by Exam Application is $75 or if you choose to get a new grad permit (NGP)-$10 extra for a total of $85.00: You will fill this application out if you sign up to take the NCLEX for the first time after graduating from an accredited LPN program. You will not be made eligible to take the NCLEX exam until you sign up with Pearson Vue: https://home.pearsonvue.com/. Before you fill out the Initial Examination Application here is the directions: Fill out and pay for the application > Get your background check through IdentoGo: https://uenroll.identogo.com/ with our code: 228NQ7 have your official transcripts sent to us from your program coordinator > Go to pearsonvue.com: https://home.pearsonvue.com/ and sign up to test > once you test the NGP expires.

* If you have paid the $10 for a New grad permit you will be issued the 90 day NGP license (New Grad Permit is good for 90 days from your date of graduation) upon receipt of your transcripts and background check. This license will expire upon taking the NCLEX.

REPEAT EXAMINATION APPLICATION-SECOND, OR MORE, TIMES TAKING THE NCLEX

Repeat Examination Application: You will fill out this application if you have failed your previous NCLEX Exam with Pearson Vue. Fill out the repeat examination application and pay $75.00 fee (If it is your third attempt our application fee is waived) > go to pearsonvue.com https://home.pearsonvue.com/ and sign up to test.

RENEWAL APPLICATION

You will fill out the renewal application every year between March 1st to June 30th by MIDNIGHT. If you fail to renew your license within this time your license WILL LAPSE, and you will have to Reinstate your WV LPN license to make them active again.

MULTI STATE LICENSE APPLICATION

To apply for a multistate application, you must hold a single state WV LPN license and live in West Virginia. If nursys.com does not show your current address as WV, then we will require proof in the form of a WV driver's license and a utility bill with your name and physical address on it.

REINSTATEMENT APPLICATION

You will fill this application out if you have previously had a WV LPN license. Before you fill out the reinstatement application these are the directions you will follow. If you have the 400 hours verification of employment you will fill out and pay the reinstatement application fee at the end > upload your 400 hours verification of employment and your 24 hours Continuing education (including the WV requirements) in the form of certificates onto your nurse portal > Obtain a background check with identogo: https://uenroll.identogo.com/ with our code, 228NQ7 > if your license says LAPSED on the nurse portal you will need to fill out a work attestation form (this form is based on the year the nurses' license became lapsed). If you do NOT see your year below call the board at 304-558-3572 so one can be created for your specific year.

Endorsement into WV

You can ONLY get a nurse licensure compact (MULTISTATE) license in the state of your primary state of residency (PSOR)

Endorsement Application: You will fill out this application if you are an LPN from another state and have never had a WV LPN license and wish to acquire one. Before you fill out the Endorsement Application these are the directions to follow. You will go to nursys.com and click on the endorsement verification and follow the directions carefully for verification to be sent to us here at WV LPN Board from every state you have held a license > Obtain a background check with Identogo: https://uenroll.identogo.com/ with our code: 228NQ7, and then it will prompt you to put in your zip code and pick an identogo near you to get your fingerprints done > If your primary state of residence has changed to WV you will need to upload a picture of your <WV driver's license and a utility bill> If you live in a noncompact state you will be issued a single state license.
